Apply for a Library Card!

Fill out the form below to apply for a library card!

Once you have finished filling out the application, you can come into the library to pick up your card at any time. Please bring an ID with you that includes your mailing address (though you could also bring a piece of mail or even an email, as long as it has your name and current address!)

When you come in to pick up your card, you will be asked to sign this form. By signing, you agree to pay any fines or damages charged to your library account, obey all rules and regulations of the library, and give prompt notice of any change of address or contact information. Failure to do so will result in loss of library privileges. Parents/guardians are responsible for fines and losses made on a juvenile’s account.
